What Is Nerve Repositioning?

Nerve repositioning is a specialized surgical procedure used when placing dental implants in the lower back jaw—especially in areas where bone loss has occurred and the inferior alveolar nerve (which controls sensation in the lower lip and chin) is in the way of proper implant placement.

This nerve runs through the lower jawbone and must be carefully moved aside to create enough space for dental implants. It is typically considered only when no other options are feasible, as it is a more aggressive approach with a risk of temporary or permanent numbness in the lip, chin, or lower jaw.

When Is This Procedure Necessary?

Nerve repositioning is considered when:

  • Teeth are missing in the area of the second premolar and/or two back molars in the lower jaw
  • There is insufficient bone height to place implants without interfering with the nerve
  • Less invasive alternatives (such as short implants or bone augmentation) are not viable

Bone Grafting in Combination with Nerve Repositioning

To ensure the implants have a strong foundation, bone grafting is often performed at the same time. Bone may be sourced from:

  • The chin or back of the mouth
  • The hip or tibia in more extensive cases
  • A donor (allograft) or synthetic graft material
  • The patient’s own platelet-rich plasma (PRP) to enhance healing and bone growth

Using the patient’s own bone generally provides the most successful and stable results.

What to Expect After Surgery

  • Outpatient procedure – Performed at Kissimmee Oral Surgery’s in-office surgical suite
  • Sedation options – IV sedation or general anesthesia to ensure patient comfort
  • Recovery time – Bed rest for one day; limited physical activity for one week
  • Temporary numbness – Mild to moderate numbness in the lip or chin is common and often subsides slowly over time
